979 studies, 14 left

A systematic review went through everything written about air pressure and migraine since 1973. The conclusion is more interesting than a yes or a no.

979 studies came in. 14 came out the other end.

That's how, in practice, a systematic review from 2025, published in the journal Cureus, on the link between air pressure and migraine begins. The authors searched the literature from 1973 onward, sieve after sieve, and ended up with 14 studies and 2,696 participants combined — aged 11 to 70, 87.9% women.

That's not much, considering how many people are certain they can feel it.

Disagreeing on direction, not on existence

The interesting thing about the 14 studies isn't that they disagree on whether there is a link between pressure and more frequent attacks. It's that they disagree on which way it goes.

Several of the studies found a link between falling pressure or rapid swings and more attacks. Others found no significant link at all. Another review, published in Current Pain and Headache Reports in 2024, gathers several of the same pieces: low pressure was linked to more headache episodes in one app-based study, while a Boston cohort found no meaningful link for either temperature or air pressure.

And then there's the Japanese study in which 18 of 28 patients (64%) got migraine in connection with a change in the weather, and 14 of them pointed to low pressure themselves as the cause. But when the monthly average pressure was held up against how often they got headaches over a whole year, no link was found.

Severity: six studies, no agreement

Only 6 of the 14 studies looked at whether air pressure makes attacks worse — not just more frequent. The results disagreed with each other. One study, using a standardised pain scale (VAS), found a significant positive link between pain and air pressure (p = 0.027). Positive — meaning more pain at higher pressure, not lower. Most of the other studies didn't use standardised scales at all, so the numbers are hard to stack against each other.

A review from 2019 sums up the problem even more directly: many studies in the field have produced inconsistent results regarding the direction itself of the link between pressure changes and headache.

The review, incidentally, gives no combined effect sizes across the 14 studies. The results stand as p-values, or simply as the presence or absence of a link in each individual study. In other words, there's no single number to sum up the whole picture in — only 14 partial conclusions pulling in different directions.

Duration: the only agreement in the whole review

Only 3 of the 14 studies looked at whether air pressure prolongs an attack once it has started. All three found nothing. That's actually the only one of the three categories — frequency, severity, duration — where the studies agree.

The review's own criticism of itself

The authors are honest about why the picture is so messy. Methods for measuring air pressure are inconsistent from study to study. Many rely on self-reporting. The samples are small and predominantly female. There are no standardised tools for measuring severity. And the geographic spread is limited.

But the last objection is the one that matters most for an app that asks you directly instead of assuming something in advance: none of the 14 studies accounts for individual variation in weather sensitivity. They treat their participants as one population with one link — or none. Not as 2,696 people who can each react differently, or not at all.

That's the piece missing from all 14 studies at once.
